Home Health Services and Pain Management

A PM&R Physician or Pain Management Physician facilitates and oversees the comprehensive pain management process and physical medicine program. The physician will then coordinate with your community’s entire medical and interdisciplinary team.

Reduction in hospital readmissions. Each client will have a social worker assigned to them before discharge. Our Licensed Social Workers will work diligently to coordinate care with each medical provider and ensure all medical supplies are obtained and follow-up appointments are arranged.

Increased rate of successful resident discharge to home. Targeted Case Management Services are provided upon discharge to assist clients with gaining access to needed medical, mental health, social, educational, and other services. 

Ongoing outcomes tracking, including:

  • Pain intensity levels
  • Medication utilization
  • Hospital readmission due to pain
  • State survey preparedness
  • Ongoing assessment of 802 matrix
  • Ongoing assessment of Casper Report
  • Annual 3rd-party audits of clinical and billing documentation
  • Onsite trigger point and guided ultrasound injections

Our certified counselors will follow up twice weekly with each client when they return home.
