Complete Intake Assessment
To receive our outpatient behavioral services, you’ll need to fill out and submit an intake assessment form. Assessments are offered via telehealth or during our walk-in hours: Monday-Friday, 8 am – 4 pm.
Meet with a Licensed Clinical Therapist
A licensed clinical therapist will be accommodating you in completing your intake assessment. Please take note—an intake assessment is only to confirm admission to our clinic. It is not a substitute for receiving medication right away.
What to Bring
Here are the things you should bring with you on your visit:
- Proof of identification and/or residence and your health insurance card
- A list of all current medications (and side effects)
- If you have a referral, please bring your copy with you.
